Changes since August on a national scale:

The latest numbers are way, way up, everywhere in the country.
President Trump had the virus in early October and was treated with Regeneron (still in experimental stage). He was not treated with hydroxychloroquine.
Here in MN, hospitals are nearly at capacity.
Just this week there was an outbreak among GOP state senators and staff, which they kept secret from the other side of the aisle.
Two new vaccines show early signs of being extremely effective, but it will be a few months before they are widely available.
